Death Certificates

Death Certificates take 1 - 2 weeks if the doctor is prompt in completing their part.
Typically 3-5 copies are sufficient as many of the more common entities will make a copy and return the certified copy to you. Additional copies can be acquired from the county health department for the first six months. after that you must obtain from the State of Oregon: Here is there Link

Death Certificates come in two versions: Long Form and Short Form. Short Form certifies the death but does not give cause of death; Long Form, certifies cause of death. The County requires a Short Form to remove a decedents name from title to real property.


Obituary

We are happy to place an obituary on our website including a photo, or even a few photos; also we will post on the Macy and Son Website In Sheridan, the Local “Bulletin Board” news letter will publish with one photo at no charge.
We can facilitate placing the obituary with the McMinnville News Register or any other Newspapers you would like. We do not charge, but the newspapers generally do charge, ask for a quote.


Social Security

We notify social security of a death when the death certificate is created. A surviving spouse is entitled to a stipend from social security of $255. This is not automatic and must be requested. A phone or in person appointment is recommended.


Cremation

Cremation takes place in McMinnville at the Macy and Son Funeral Home. Cremated remains are generally ready for you to pick up at the Sheridan Funeral Home in 5 or less days. Your designated family representative will be notified when they are ready. We provide a selection of Urns for an additional charge. If you bring to us third party urns, we do reserve the right to not fill them for you in some instances. For more information, please enquire


Veterans Benefits

Veterans are generally entitled to a flag and military honors at their service, regardless of location; at a non-national cemetery they are also entitled to a grave marker. Additionally if desired, the veteran and spouse may be buried in a National Cemetery. HERE IS MORE INFORMATION


Funeral Home Charges

All charges are due and payable at time of arrangements. Charges may be paid by check, CC or cash. If there is an Insurance policy, after verification it may be accepted for payment with an assignment to the funeral home. There is a charge for the assignment as we often must wait for payment. If a pre arrangement has been done, that will usually care for all of the charges for services that were planned for. We will go over all the details with you at the arrangements meeting. Services are only rendered after satisfactory arrangements have been made.
